Instructions:
1. Click the link above.
2. When prompted, sign in with either your phone # or email. You will be sent a sign-in code.
3. Fill out form.
4. Hit submit.
If filling out for another individual in your family, open form  again and select them from the list. Fill out info and hit submit.
Note: if you don't see the individual in the list, scroll down to the bottom and select "add child" or "add adult"
